If you are a CNA, RN, or LPN with a good working history, our team would love to hear from you. Below we've compiled a list of the required documents that you'll need to get started. Once you have everything in order, just call (410) 719-0672 to schedule an interview with a member of our team.
Have the Following Things With You for Your Interview
- Original State or Federal photo ID
- Original Social Security card
- Alien card / permanent resident card
- Current nursing license (verified on MBON website)
- Current CPR card
- First aid card
- Malpractice insurance
- Two work references (name, mailing address, and phone number)
- Two personal references (name, mailing address, and phone number)
- Physical exam (performed within 6 months of application)
- Hepatitis B record or declination form
- PPD (within 1 year) or chest X-ray
- Background check (optional for CJIS only)
Applicant Requirements
Please be sure to provide the following documents. Employees may not start or continue with assignments if any information is incomplete, invalid, or missing from the employee file.
- Hard copy of a negative PPD / chest X-ray. PPD's must be within one year and need to be done annually and forwarded to the office, and a chest X-ray
- A criminal background check form will be provided to you
- Please provide: (HR will make a color copy to be placed in your file)
- State ID / driver's license and employment authorization card (original)
- Social Security card (original)
- Alien card (if applies)
- Nursing license (CNA, CMT, RN, LPN)
- CPR card and first aid certification (valid and up-to-date)
- Malpractice insurance (bonding letter)
- Other certifications, degrees, diplomas, etc.
- References: 2 business and 2 personal (complete information: contact person, company name, address, and phone numbers)
