REQUESTING TIME OFF & RETURN TO WORK
Procedure:
- All time off/leave requests for Nurses and CNA’s must be submitted in writing to the staffing coordinators or via the Compassionate website at least 30 days in advance, if requested time off is 7 days or more.
- Nurses who work on the same case and have overlapping requests, will be awarded time off on a first come, first serve basis. Multiple nurses will not be allowed to request off for the same time if they are working the same case.
- If the leave request is less than 7 days, at least 1-week advance notice is required.
- Start and return dates must be clearly written on requests
- Once a time off request is received by the staffing department, a confirmation of the receipt of the request will be sent to the person requesting time off.
- The requesting party must have received a notification of approval from the Staffing Department before the request for time off is considered granted.
- Upon return, the independent contractor must contact the staffing coordinators within 24 hours before he/she is scheduled to resume work.
- If the independent contractor fails to contact the staffing coordinator, then he or she is not authorized to return to work.
- Violation of this policy may result in disciplinary action.
